Why ETS for Facial Sweating Remains Controversial
The uncontrolled and embarrassing situation of facial sweating and blushing was thought to be easily treated with ETS. This came about when ETS was done for patients with sweaty hands. Among those patients who also suffered from facial blushing and/or facial sweating the results were also successful at reducing facial blushing and/or sweating. However over the years two observations were made when this operation was applied only for patients with facial blushing and or facial sweating. Percentage wise these patients developed a higher rate of severe compensatory sweating.
Also these patients experienced a higher degree of dissatisfaction due to the side effects. Side effects such as facial flushed feelings, loss of stamina, facial skin sensitivity, increased amount of fatigue and others led Dr. Basseri to the decision not to perform ETS when only facial blushing or facial sweating were involved. The clinical experience that was accumulated over the last several years is what has allowed Dr. Basseri to reach this assessment. More time is needed to reach a definite conclusion with regard to the most appropriate procedure for patients who suffer from only facial blushing and/or facial sweating.
ETS for Facial Sweating or Blushing
A notable portion of our patients — approximately 10–15% — also experience facial sweating, facial blushing, or both alongside their primary concern of sweaty hands (palmar hyperhidrosis). In these cases, we are able to modify our ETS procedure to simultaneously address facial symptoms, offering a comprehensive solution in a single treatment.
When facial sweating or blushing is treated as part of the modified ETS procedure, patients can expect an excellent outcome, with a success rate of over 90%. This modification is seamlessly integrated into the procedure, providing relief for both the hands and face at the same time.
It is important to note that we do not perform the ETS procedure for patients whose sole concern is facial sweating or blushing.
Sweaty palms (palmar hyperhidrosis) must be the primary indication for the procedure. For patients who are candidates, however, addressing facial symptoms concurrently can greatly enhance their overall results and quality of life.
What We’ve Learned
Why ETS Isn’t Recommended for Isolated Facial Sweating Or Blushing
Experience has shown that patients who underwent ETS solely for facial blushing or facial sweating tend to report a higher degree of dissatisfaction. While many patients achieved successful results, a significant percentage developed severe compensatory sweating leading to dissatisfaction. This insight emerged over time through careful patient follow-up. Due to the increased rate of dissatisfied patients, Dr. Basseri does not perform this procedure when facial blushing or facial sweating is the patient’s only concern.
